Comments, Opinions and Reviews Carotid Artery Noninvasive Testing is Being Overused

چکیده

The most frequent tests done in the evaluation of patients with potential cerebrovascular disease are Oculoplethysmography (OPG) and Imaging procedures. These procedures are useful in the evaluation of asymptomatic carotid bruits and may be helpful in evaluating patients with nonhemispheric symptoms. However, there is no reasonable indication for the use of these noninvasive procedures on patients with lateralizing hemispheric or ocular transient ischemic attacks. Lateralizihg transient ischemic attacks are very often caused by ulcerated non-stenosing lesions which are easily missed by these noninvasive means. Arteriography is mandatory in this group whenever the patient is a potential surgical candidate should an appropriate lesion be found. Finally, progress made in bruit analysis is acknowledged but the clinical relevance of this datum is questioned.

Comments, Opinions, and Reviews Spontaneous Internal Jugular Vein Thrombosis and Venous-Stasis Retinopathy

A case is presented in which the detection of venous-stasis retinopathy in one eye led to investigation of the carotid circulation. There were no neurologic symptoms of carotid insufficiency, and noninvasive tests failed to reveal significant carotid pathology. Digital subtraction angiography and carotid angiography demonstrated a carotid plaque of doubtful significance. At carotid endarterecto...

ارزیابی و بررسی نتایج حاصل از داپلر فراصوتی در مقایسه با آنژیوگرافی تفریقی دیجیتال در تشخیص تنگ شدگیهای شریان کاروتید داخلی

Introduction: Digital Subtraction Angiography (DSA) is the standard method for detection of carotid artery stenosis. This technique has high radiation dose. With the progress of imaging techniques, noninvasive or minimally invasive methods like the CT angiography, MR angiography and ultrasonography with Doppler or Duplex mode were applicable. This study aimed to evaluate and assesse the results...

Noninvasive testing for carotid artery stenosis: I. Prospective analysis of three methods.

Oculoplethysmography-carotid phonoangiography and periorbital directional Doppler sonography are two techniques widely used in the noninvasive evaluation of possible carotid artery disease. Recent advances with sonographic Doppler devices now permit measurement of blood velocities in the extracranial carotid arteries by direct scanning with a color coded Doppler imaging system. A prospective st...

COMMENTS, OPINIONS, AND REVIEWS Chronic Ocular Ischemia And Carotid Vascular Disease

Venous stasis retinopathy and ischemic oculopathy are ocular manifestations of ischemia in the distribution of the carotid artery. While not as common as transient monocular blindness or retinal arterial emboli, they are readily recognizable and indicate the presence of severe, often bilateral, carotid occlusive disease. Patterns of occlusion vary but usually include complete occlusion of at le...

Complementary roles of a noninvasive test battery and DSA in evaluating carotid artery disease.

Initial comparisons of the results of digital subtraction angiography (DSA) and a battery of tests for noninvasive diagnosis of carotid artery disease indicate the two techniques are complementary, rather than competitive. DSA provides important morphologic information at the carotid bifurcation and siphon, but the images are sometimes difficult to interpret with precision. Noninvasive testing ...
